What the price is actually buying

Why does the same capability cost $38 a month from one vendor and $600 from another?

Because you are not buying the same thing. The self-serve price buys a product you configure yourself, deployed alongside the chart, with support by email. The enterprise price buys native EHR integration built by the vendor's engineers, IT governance, a signed BAA at institutional scale, implementation services, and someone accountable during a rollout across thousands of clinicians. Whether that difference is worth fifteen times the price depends entirely on whether you have an IT department that would otherwise do it.

Should price be part of a clinical AI evaluation at all?

It has to be, and the reason is not budget. A tool a single clinician cannot evaluate without a sales process gets adopted differently from one they can trial on Tuesday, which changes who selects it and on what evidence. Every rubric on this site weights access — published pricing, free tiers, credential and regional eligibility — equally with capability dimensions, and that weighting is why free global tools frequently outrank expensive curated references here. An institution with an existing licence should re-rank without it.

What is the real cost of a free clinical AI tool?

Depends which kind of free. Advertising-funded means a third party pays for adjacency to clinical decisions, which is a governance question rather than a quality one. A free tier alongside paid plans usually caps volume, templates or specific features, so the thing you evaluate is not quite the thing you would buy. And free tools tend to lack the BAA, audit logging and single sign-on that make institutional deployment possible, which is a cost that arrives later rather than one avoided.

Published list prices are a starting point, not what an organisation pays. Enterprise contracts are negotiated against seat count, term length and integration scope, so figures described here as frequently cited are directional rather than quoted. 13 of 35 tools publish no rate at all, which means any total cost comparison across the whole field is incomplete by construction. Prices also exclude the costs that decide real deployments — implementation, training, IT time and the licences that sit idle after month three.
